The other tenderers submit:
• The transcript of the education document and copy;
• Family health certificate;
• if a person's name changes – a change of surname to deliver a copy of a document;
• a statement regarding the declaration of the place of residence;
• Passport or a copy of the birth certificate;
• Persons with disabilities – professional suitability evaluation recommendations;
• Four photographs (3 x 4).Receipt of services
