Social Integration State Agency provides social rehabilitation services. Social rehabilitation is a set of activities that helps to learn how to cope with functional impairments, restore or acquire skills that were lost due to various circumstances, help people to return to work and angage in social activities. It includes the development of an individual social rehabilitation plan, training in the area of the environment adaption and aids to improve mobility and independent life skills.

The duration of the social rehabilitation service is from 10 to 21 days.

To receive a service:

1. Persons with functional impairments can get 14 days long social rehabilitation course after state-paid multi professional medical rehabilitation in the hospital or day hospital:

  • if the disorders caused by the disease are at the level of activities and participation according to the International Classification of Functioning, Disability and Health;
  • if the medical institution in the excerpt of received medical rehabilitation has recommended to receive social rehabilitation service and its goal.

In order to apply for social rehabilitation, within two months after the medical rehabilitation course is completed an individual must submit to the municipal social service:

  • application with a request for service provision;
  • completed Performance Capability Self-Assessment Questionnaire;
  • excerpt (copy) of received medical rehabilitation with recommendation to receive social rehabilitation service and its goal.

The service can be received with an accompanying person only if:

  • the severity of the person's impairment according to the Bartela index has been assessed from 1 to 6 points;
  • the person with functional impairments receives an allowance for a disabled person who needs care;
  • the person has limited capacity due to which he or her needs supervision and care.

If individual can receive social rehabilitation service only of an accompanying person presence, when applying for the service, in the persons documents must be included an application or another document that attests the readiness to pay the expenses related to the stay of the accompanying person in SIVA.

After receiving the documents, the municipal social service specialists checks whether the documents have been submitted in accordance with the regulations and assesses the person's self-care and mobility abilities according to the Bartel index and then sends the documents to SIVA.

SIVA specialists evaluate received documents and make a decision about service assigning or refusing and sends it to the person and the municipal social service (for information).

2. Persons with anticipated disability can receive 14 day long social rehabilitation course, if in the individual rehabilitation plan approved by the State Medical Commission for Health Condition and Work Ability specifies the need to receive social rehabilitation.

In order to apply for social rehabilitation person must submit:

  • application with a request for service provision;
  • an individual rehabilitation plan approved by the State Medical Commission for Health Condition and Work Ability and a copy of the finding about assigning the status of anticipated disability.

SIVA specialists evaluate received documents and make a decision about service assigning or refusing and sends it to the person.

3. Individuals who had been persecuted for political reasons and victims of Chernobyl disaster can receive 21 day long social rehabilitation course.

In order to apply for social rehabilitation person must submit to the municipal social service or in SIVA:

  • a passport or an ID card must be provided;
  • application with a request for service provision and a copy of the document attesting their status;
  • the extract issued by the general practitioner from the persons medical card about individuals health condition, in which information about diseases that require special attention and use of certain rehabilitation technologies is indicated.

Municipal social service specialists assess received documents and then sends them to SIVA.
SIVA makes a decision about service assigning or taking a person in a queue for receiving a service.
The service is assigned 
within the queue order of previously mentioned group.

Receiving the service depends on the individual needs.
